Very often when Mom's behavior and even sometimes cognition changed, though she did not have any symptoms of a UTI, it was indeed a UTI of the "silent" variety. Once she was treated, she was back to baseline.

CAVEAT: Several times my mother had a course of antibiotic therapy but things were still off kilter. When I insisted we re-check the urine, sure enough; she still was with positive results. Twice she had to receive IV abx followed by a more lengthy course of oral abx in order to quell the nasty organism.

Sometimes it's easy to forget about UTI's, but over the years I finally got to the point where I'd think "UTI" first and then other causes later when things went over the top.

Absolute ditto on this one. And who the heck would think that one end would effect the other so much!? Before mom's speech went all together, I *knew* when she was starting with one because her speech would change. Words would become a lot more garbled. She was getting, on average, one a month and too many times after finishing the anti-b, she still wouldn't act right. It would then take going to the hospital for IV anti-b treatment. The nurses would ask me how I knew mom was getting another one and I'd tell them "listen to her talk". They finally picked up on it and if they heard a change, they'd test her again for one.

Right now she's on a maintenance dose, but she's still coming down with them, albeit a bit more slowly than before the maintenance dose.

Thanks, JAB!
I just read this new thread from yesterday:
" To Nessa and Everyone Else..."

which has more info on UTIs and in-home dip sticks. It is important to try and have the urine cultured for the infection, to identify the bacteria, if present.

Since most infections are caused by Escherichia coli (E. coli), doctors usually prescribe an antibiotic immediately, to treat this bacteria, until the culture comes back, with either confirmation of E. coli, or another bacteria.

"Some microbes, like Chlamydia and Mycoplasma, can be detected only with special bacterial cultures. A doctor suspects one of these infections when a person has symptoms of a UTI and pus in the urine, but a standard culture fails to grow any bacteria."(nih)

What do you do when your mother's behavior becomes different than it was and the doctor doesn't care?
Mom woke up several days ago and usually she has a stack of "things" she plays with, but now she seems almost psychotic. She babbles constantly, she get's up wanting to get out of here. She can barely get up, but she tries her darnest. I've increased her ativan and started her a few days ago on Seroquel, but so far no change. I feel something could be wrong physically, but when I called the doctor he said that's how it is with AD, they can stay awake for days. Mom is suffering and I'm afraid. What do I do?

If you aren't happy with her doctor, maybe it's time to find a new one?
Is this her primary care doctor or a psychiatrist/neurologist. Most primary care doctors aren't too great with dealing with things related to AD.
If this is a sudden change for her, her doctor should have ruled out other causes.
If her doctor isn't helpful, you might want to consider taking her to the emergency room (assuming that these are new changes) to rule out non-AD things. And, if it is AD, they could admit her to a psychiatric hospital for medication stabilization.

This is what I asked the doctor about, sending her someplace to get her meds under control, but he just doesn't seem interested.
This doctor is an AD specialist, I live in a small town where there is no other specialist, he's it. However, it seems like he takes it all for granted and just brushes everything off as AD related and doesn't even check her. I took her to the ER about a month ago when I thought she was having a heart attack as her neck was bulging every time her heart beat. They didn't do a cardiogram even. I wanted two hours before the doctor came in at all, it's like they figure she's old, why bother. I wish I could find good care for her, but I don't know where that's going to be at.
I've asked the alz association for a list, but the list is just that. I can't get out to look as I'm her only caregiver. This is new behavior, she is yelling and seems like she has no clue what's going on. If I give her the things she likes to play with, she just throws them. She won't eat or drink much and is afraid of the food. Yes, this is a big change, overnight it seems.

Jean, would taking her to an ER in a bigger city be an option for you?
I wish I had better advice, I wish she had a better doctor. This could be something as simple as a urinary tract infection.

One more thought, what about taking her to a primary care doctor. Maybe they would look at physical possibilities for her sudden changes?
I'm so so sorry that this "AD specialist" just plain sucks.

I left her primary care doctor because he was just the opposite of this doctor, he was always putting her in the hospital, always on the heart floor where she was constantly being picked at. He ordered all kind of tests without even discussing it with me and mom could barely get through one of them.
This doctor won't do anything, he takes the keep them comfortable approach, but he doesn't even do that.
Your right, this does suck and now for the first time as she keeps shifting around says that it hurts so bad. I don't know if it's a UTI or what hurts, but she's keeps yelling and she usually doesn't do that. Thanks for your help. I'm getting another doctor, but I guess I'm going to have to go out of town to do that.

Jean,
With this pain, I would definitely take her to the ER now, even if it's an out of town ER.

Does anyone know if Medicare will pay if you take the person to another state to be seen or do you have to stay in your own state?

I think they still pay, I'm not positive.

Keep in mind, the symptoms in our ADLOs could be drastically different, than in a "healthy" individual.

They can include:
Delusions
Hallucinations
Rage
Frustration
Anger
Confusion
Withdrawing into one’s self
Refusing to eat/loss of appetite
Change the way they walk/inability to walk
Complaining of pain in other parts of body
Bed/floor wetting

ALSO, insist the urine is cultured, to identify the offending bacteria.

It is important to note that any infection, constipation, dehydration, medicinal reaction, hidden injury, etc., should also be checked for causes of changes.
